OpenOffice is a great free substitute for Microsoft Office and will allow you to complete our forms and email the file in Excel “XLS” format back to us as a file attachment once you complete the forms. OpenOffice comes in versions for both PC and for Mac (NeoOffice).  Click below to download the version needed for your computer.

Once you have downloaded OpenOffice, please follow the instructions for “Excel” on the previous page to dowload our “XLS” file to use. OpenOffice is a complete “Suite” of wordprocessing and spreadsheet programs. The spreadsheet program to use our XLS file is “CALC.” When you save the file in Excel XLS format in the “CALC” program , you will receive a warning that you will lose password protection. That is not a problem. Thanks for taking the time to do this! Be sure to attach the completed file to an email and send it to us at:
